CMV, like the herpes viruses to which it is related, causes infection in many people, lies dormant after symptoms disappear, and can recur later. Like MAI, CMV infections are spread throughout the body, almost always when the CD4 count is less than 200, usually less than 100. In the liver, CMV causes hepatitis; in the intestines, diarrhea; in the esophagus, difficulty swallowing; in the lung, pneumonia; on the skin, herpes-like sores; in the eye, retinitis. In many people, however, CMV infection only causes constitutional symptoms, including fever, fatigue, and wasting.

One of the most serious of these infections is CMV retinitis, which, if left untreated, can cause blindness. CMV is transmitted the way HIV is, through sexual contact or contact with blood. Most people do not know when they were infected, and the initial infection usually does not cause symptoms. For these reasons, CMV is not considered a health threat to others.

CMV infection is difficult to treat: like MAI, it resists most antibiotics. The drugs

used—ganciclovir and foscarnet—must be given intravenously. CMV retinitis responds well to treatment; CMV at other places in the body responds less predictably.

A condominium is an apartment or townhouse that is privately owned, usually by the occupant. All the rules that govern the purchase of a single-family house apply here. The difference is that condominiums normally have a governing body of owners that determines policies affecting each unit and the common areas. In some cases condominium boards have discriminated against individuals based on their disabilities.

If your condominium is a townhouse with several different levels, ensuring mobility will be challenging. One option might be to center your primary activities on one level. Another choice is to install either an elevator or a stair-climber. A stair-climber is a device attached to a railing on the wall; it may be a chair in which you sit or a platform on which you ride in your wheelchair. Some power wheelchairs are specifically designed to climb steps.

Some people with tight finances stay in their multilevel homes and bump up and down the steps on their posteriors. For this exertion, however, you need good arm strength and balance, and you must frequently assess the wear and tear on your backside to guard against skin breakdown.

Condominiums have the advantage over free-standing houses of monthly fees that cover the maintenance of lawns and parking lots and snow removal. And they have an advantage over rental homes in the usual tax advantages of home ownership. On the down side, you need to anticipate escalating condominium fees, which may eventually become unaffordable.

It is up to you and your child to tell friends about the epilepsy. The information conveyed, and the persons to whom it is conveyed, should depend on the type and frequency of the seizures. For the child with absence or partial complex seizures that occur with some frequency, it is far better for the friend to understand the seizures than to assume that the child is “weird,” on drugs, or going crazy. One of our teenagers with unrecognized absence seizures had been nicknamed “Spacey” by her friends. It is more useful for a friend to understand what to do should a seizure occur and not to panic.

Unless the seizures are frequent, it is unnecessary to inform everyone with whom your child comes in contact. However, his good friends and families with whom he spends time should be aware of his seizures and how they are controlled. In general, the fears and apprehensions that are consequences of ignorance are far worse than any reality.

“Should I tell my date?” is a question we are often asked by teenagers. Again, this depends on the frequency and type of the seizures. Certainly if a relationship begins to become serious, your teenager should be self-confident enough to talk openly about his or her condition.

Over-enthusiasm, strain, tension.

Vervain stands for the soul quality of self-restraint. In the positive Vervain state the person has full control on his passionate zeal to work. Even when he is imbued with a keen desire to work at breakneck speed without a break, his Superior Self holds him back for some rest, at times to recoup his energies. This prevents total breakdown of his health. However, in the negative VERVAIN state, the person loses contact with his Superior Self. There is thus no restraining force which can call a halt at any stage. By his nature a VERVAIN type is over-enthusiastic in his mission. When he works, he works non-stop. He is goaded in his mission by an innate desire to prove himself one up, so that others may follow him. He works at break-neck speed, works non-stop and ceaselessly without rest.

His over-zealousness is manifested in all his actions. If he has to drive a nail in the wall, he would not mind using a sledge hammer for it or use force far in excess of what is required for the job.

If he is working for a political party or some social organization, his involvement is total. It is often more than that of the person who introduced him to the organization. His over-zealousness sometimes runs counter to his own interest. When he tries to convince others in a missionary way-using all types of arguments one after the other, the other person may feel harassed and bored rather than interested, and there the story ends.

Mr.D.S. Narula was a man of principle. He did not interfere in others’ work but he could not be digressed from his chosen path under any circumstances. A true Godfearing man in a Government department where corruption was rampant, he did not accept any bribe and would not act as a conduit to transmit any bribe to his officers—result, he was appointed as an overseer and retired as an overseer. He could not afford to marry and remained a bachelor through out his life.

During the course of his service he had not created any friends. The contractors, his subordinates and his officers against whom he had been reporting for inefficiency and bribery, as a matter of principle, were not well-disposed towards him, although in their inner most hearts they feared him and yet respected him.

His integrity, truthfulness and fearlessness, his devotion to duty and a constant endeavour to improve his image so that others would follow him may have earned him their approbation and respect, but few would follow his lead.

On retirement, he found himself lonely and despondent. He was suffering from frequent heaviness of head, pain and stiffness of knee-joint, and a new fear assailed him—who would attend to him if he became incapacitated due to joint-pains. The thought irritated him and he showed this irritation by becoming harsh on his servant who had threatened to leave the job. He was prescribed ‘Impatience* for irritability, “Mimulus” for fear and “Rock Water” for his inflexible nature.

After one week ‘Impatience’ and ‘Mimulus’ were discontinued because his irritability and fear had become things of the past. ‘ROCK WATER’ was continued T.D.S for another 8 weeks, to remove negative-ness and fool-hardiness in his otherwise principled life. Other physical troubles like heaviness of head and pain in the knee-joint subsided automatically.

Approved for use by the FDA in 1990 and marketed since February 1991 for use in the United States, Norplant is one the newest forms of hormonal contraception. It has been tested by more than 1 million women in 45 countries and is now approved for use in 14 countries. Increasing numbers of women in the United States are considering this option because of its convenience, effectiveness, and safety.

In this form of contraception, six silicon capsules that contain progestin are surgically inserted under the skin of a woman’s upper arm. For five years, small amounts of progestin are continuously released. The progestin in Norplant works the same way as oral contraceptives do; it suppresses ovulation, prevents growth of the uterine lining, and thickens the cervical mucus.

Norplant is one of the most effective methods of birth control ever developed. A serious disadvantage to Norplant use, however, is its lack of protection against STIs.

Norplant can be inserted by a specially trained doctor, nurse, or nurse practitioner in 10 to 15 minutes. A local anesthetic is administered to the upper arm, a small injection is made, and with a special needle, the six capsules are placed just under the skin in a fan shape. The capsules are similarly removed after five years or, if necessary, at any point after their insertion.

The capsules usually cannot be seen, nor does insertion leave a scar for most women. At this time, no serious side effects are known. Less serious side effects include irregular bleeding and irregular menstrual periods, acne, weight gain, breast tenderness, headaches, nervousness, depression, and nausea.

Norplant is one of the most effective reversible methods of fertility control. In addition to being very convenient, the implant is easy for a trained practitioner to do, so there is little chance of error. It costs less than the pill – 550 dollars compared to 1,180 dollars over five years. Medical assistance programs in many states will pay this cost for poor women.

Hypertension is more prevalent in people with diabetes than in age-and gender-matched nondiabetics. In type 1 diabetes, an elevation of blood pressure is usually seen concomitant with the development of microalbuminuria and is present in 18-33% of people with type 1 diabetes. Hypertension is at least twice as prevalent in people with type 2 diabetes than in nondiabetic individuals and is a frequent finding in the metabolic syndrome. African Americans have a very high prevalence of hypertension. Elevated blood pressure is a major contributor to cardiovascular mortality and morbidity, renal failure, retinopathy progression, strokes, peripheral vascular disease, and amputations in diabetic subjects.

In recent years, there has been an explosion of information from large scale collaborative clinical trials that aggressive treatment of elevated blood pressure in people with diabetes has major benefits in preserving renal function and reducing the incidence of cardiovascular events. These studies are reviewed in detail in another chapter.

Presently, the consensus between the American Diabetes Association and the National Kidney Foundation is that the goal for therapy in adults with diabetes is to achieve a blood pressure < 130/80 mmHg. The ADA suggests some modification of these goals in patients with isolated systolic hypertension.

Even if your child has a number of seizures, and even if it is difficult to find the best medicine to control them, there is still a substantial chance that they will be controlled. There is also a very good chance that after they are controlled for two years, medicine can be discontinued. Your child will have outgrown his seizures. The seizures of 80 percent of children will be controlled, and most of those children will cease taking medication. Thus, most children will not have epilepsy forever.

Even with these encouraging thoughts, when your child has had recurrent seizures, many emotions come into play. It is not uncommon for a person to vacillate between fears, grieving, and anger. The first emotion most people experience is fear.

Coming to Terms with Epilepsy: Fear and Grieving

Fear is parents’ normal response to hearing that their child has epilepsy. It is really fear of the unknown. Information helps make the unknown known. Reality is always less frightening than what you imagine.

Grieving is another emotion that parents must go through when faced with epilepsy. You are sad and grieve for the child whom you think no longer exists as before. You grieve because of your concerns about what epilepsy might do. You grieve for yourself because you’re embarrassed and fearful. You grieve because the seizures may present difficulties. You will now have to take time to go to the doctor; it will interfere with your job, your activities, your interests. All of these feelings are common and normal. Indeed, we worry about the parent who shows no signs of grieving. You have to go through grief before you can achieve acceptance.

How do you cope with this grief? It’s all right to cry, to feel sorry for yourself and your child. You need to realize that you may not be able to cope all alone. Grieving is much easier when it is shared with someone else. Another person can help you put your grief in perspective. You need to reach out and find support. You may find this within your own home by talking about your fears and concerns with a member of the family. You may find it from another parent or group of parents. The Epilepsy Foundation of America or your local affiliate may be able to put you in touch with resources that will help you deal with these feelings. While grieving is a normal stage, it has to come to an end and ultimately be replaced by a more productive approach to living with epilepsy.

Cleansers

The purpose of cleansers is to remove dirt and make-up from the skin. They do not remove blackheads or whiteheads. There are three types of cleansers available and these are suitable even for the eye area unless they cause stinging.

Soaps

It is often said that soaps should not be used on the face. Although this is true of the older, harsher soaps, milder soaps can be safely used on most skin types. Soaps are very effective for cleaning and degreasing the skin. Some soaps, however, may irritate the skin because they remove its protective fatty layer. Milder soaps are less irritating and can be used daily on the face. Examples include Almay facial soap and Clinique and Neutrogena soaps. Moisturizing soaps, such as Aveenobar and Dove, are also available and are good for those who suffer from dry skin.

Liquid cleansers

Liquid cleansers may come as lotions, foams or gels. These also remove the dirt and make-up and function in much the same way as soap. They are very safe to use and are very effective.

Some good products are Cetaphil Lotion, Yardley foaming wash, Neutrogena liquid cleanser and Avicide.

Cold creams

Cold creams should only be used on very dry skin or for removing very heavy make-up. Although cold creams do remove dirt, they leave a film of cream on the skin. If this is not washed off acne, blackheads and whiteheads will appear after several months. Cold creams should only be used by more mature people and under no circumstances by people with oily skin or acne.
